Whether we’re working on the frontlines in Programs or working behind the scenes in Support Services, we feel privileged to use our education, experience, and gifts to love people and bring glory to God.   The Case Manager for the San Jose Women’s Program at House of Grace will be an integral part of our team, providing structure, accountability, stability, and hope for the women in our residential program. The Case Manager is responsible for ensuring safe, quality, professional, and spiritual nurturing of our residents and guests. The Case Manager is responsible for following through on case management procedures and policies for the residents living onsite. This includes data entry, creating reports, and sharing front office administrative tasks. In addition to helping residents with step work and working through their program modules, the Case Manager shall always be concerned with their spiritual welfare. Our environment, which is often filled with laughter, joy, and praise, can often be unpredictable. We are looking for a Case Manager who can pivot quickly, handle disappointment and surprises with grace, and handle conflict in a Christ-like manner. In addition to working cooperatively with others, the Case Manager will need to work with limited supervision, always keeping safety and supervision a top priority. The Case Manager is a full-time non-exempt position. All of this is wrapped up in a Christian environment that values Teamwork, Accountability, Humility, Innovation, and Joy.  Essential Functions Provide supervision for residents and supervise discipline in the area of general behavior and program rules. Oversee residents’ in-house job placement, training, and performance evaluations. Monitor daily program schedules for interns, clients and volunteers. Interview residents, review records, and confer with other professionals in order to clinically evaluate individuals’ mental and physical condition, and to determine their suitability for participation in a specific program. Conduct phone screening and intake new applicants as needed. Confer with family members and others close to residents in order to keep them informed of treatment plan and progress. Maintain a caseload of residents and ensure charts are up to date by performing weekly audits. Complete and maintain accurate records and reports regarding the residents’ history and progress, services provided, and other required information. Assist and keep track of residents’ legal issues relating to court, probation, and other legal issues as they arise. Intervene as an advocate for residents in order to resolve emergency problems in crisis situations. Network and interact with various county and private agencies to secure needed services and benefits on residents’ behalf. Serve as liaison to various vocational rehabilitation services and facilities to insure residents’ timely transition into the vocational component of the program. Provide spiritual leadership and counsel to residents including counseling one on one, group counseling, and planning in-house and outside activities as needed. Perform administrative, front office duties which may include answering the phone, replying to emails, and data entry. Train and coach interns, residents, and volunteers in performing office and program responsibilities. Coordinate resident activities, including facilitating small groups, Bible studies, and classes. Conduct a series of life skills training classes designed to enhance client’s skill levels in a variety of practical areas (i.e., budgeting, parenting, cooking, etc.). Work collaboratively with the Development Team, specifically the Community Engagement Manager & Community Engagement Coordinator to plan, schedule, and host positive experiences for volunteers and potential donors. In conjunction with the counseling and CityTeam @ Work staff, provide adequate supervision and guidance related to employment counseling, skill acquisition and placement. Work with Support Services to meet all requirements and paperwork in a timely manner according to the guidelines established by HR & Finance. Attend and participate in related staff meetings, trainings, and resident review sessions. Be available to transport clients when needed. Follow progress of discharged clients in order to determine effectiveness of treatments. Advocate for CityTeam in your circles of influence and support CityTeam’s fundraising strategy and events. Other duties and special projects as assigned by the Program Manager.   PLEASE NOTE: To perform this job successfully, an individual must be able to perform each of the essential duties satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.   Required Education and Experience Degree in Social Work, Counseling, or equivalent experience Computer literate and proficient using Microsoft Office Suite and or Google Suite, especially Word and Excel Familiarity with CityTeam, Disciple Making, and addiction recovery principles Good interpersonal skills, with proven ability to communicate with people from diverse backgrounds Must have a valid California driver’s license with a clean record   Preferred Education and Experience 3+ years’ experience in case management or related professional experience CCAPP certification; CADC-1 minimum Supervisory experience including setting goals, performance management, and progressive discipline Facilitation of interactive process group sessions Experience and specific course work in the area of chemical addiction, codependency, crisis intervention, and family dynamics Good written communication skills, experience writing court reports and or assessments   Physical Demands Stand or sit for prolonged amounts of time Walk, bend, twist, crawl & kneel Talk, hear, & smell Occasionally lift and move up to 25 pounds Finger and hand dexterity to feel, handle, and operate objects, tools or controls, and to reach with hands and arms Specific vision abilities required by this job include close vision, distance vision, peripheral vision, and depth perception Drive   Work Conditions This job operates in a professional office environment and residential apartment setting This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines This position will typically work Monday-Friday with occasional evenings and weekends The exact hours will vary depending on ministry scheduling needs. Project Management: Proactively manage calendars, schedules, and events. Create timelines, action lists, and keep everyone on track. Proofread, revise, and edit emails, collateral proposals, letters, and PowerPoint presentations. Utilize technology efficiently (Raiser’s Edge, Qgiv, Volunteer Hub, Campaign Manager) to strategically use data to drive action.  Work with the VP of Development, Marketing, and the rest of the Development Team to ensure appropriate information and materials are made available to donors. Advocate for CityTeam with those in your circle of influence. Handle all information with the strictest confidence.
