Major Duties (75% to 80% of Job):     

1. Recruitment, Admissions and Discharge Planning
• Ensure new resident admissions requirements are followed as outlined in DHS 83 and 89.
• Serve as the primary point of contact for initial prospective resident inquiries, providing information, and conducting tours as needed.
• Facilitate the prospective resident application process, including completion of initial application; acquire records and complete comprehensive assessment (medical, physical, psychological, behavioral, and financial) in collaboration with Nursing Department leadership; assist prospective residents, families and/or guardians as needed to complete applications and other requirements related to long term care funding, insurance, and other needed services; complete new resident service agreement, risk assessments as needed, and other state-mandated admissions documents prior to admission.
• Conduct organizational orientation for all new residents, including new resident welcome and onboarding, and pair new resident with a “resident buddy.”
• Complete all new resident 90-day regulatory requirements on time.
• In collaboration with nursing leadership and other key staff, lead the resident discharge planning process.
• Create and effectively execute an aggressive marketing and new resident recruitment strategy to attract prospective residents.


2. Liaison to local, state, and federal programs, managed care organizations, and other resources
• In order to ensure coordination of care and optimal quality of life for residents, maintain thorough understanding and working knowledge of the local, state and federal systems and structures that exist to benefit the frail/elder and disabled populations and connect, refer and facilitate connection with resources and entities including, but not limited to:
i. Aging and Disability Resource Center
ii. Managed Care Organizations/Family Care Program
iii. Board on Aging and Long-Term Care (Ombudsmen)
iv. Department of Vocational Rehabilitation
v. Department of Workforce Development
• Proactively collaborate and grow fruitful relationships with case managers from Managed Care Organizations with which Inspiration Ministries is contracted.
• Develop and grow relationships with external resources, services, and/or programs that will improve the spiritual, emotional, physical, and behavioral wellbeing of residents.

3. Resident Services
• Serve as the point of contact and lead resident Behavior management efforts and work as needed with residents and staff from appropriate functional areas to resolve resident conflict.
• Service Plans (ISP) – On an annual basis (or when there is a change in condition), create and update comprehensive, customized care plans for residents that are based on their individual interests, needs, and desires.
• Thrive Plans – On an annual basis, facilitate the creation of resident-centric plans that help each person achieve their goal(s). This will be accomplished by leveraging, when needed, internal resources (nursing, dietary, resident life/activities, etc.) and external, clinical, and non-clinical partnerships (behavioral health partners, therapy service partners, and more).
• Resident Satisfaction Survey process – Annually administer the Resident Satisfaction Survey, lead Operations Team through review of survey results and the creation of tactics and strategies that result in improved scores for their respective functional areas.
• Resident benefit renewals and advocacy – Including Medicaid renewals; Supplemental Security Income (SSI) verification; Medicare renewals, upgrades, changes; Department of Vocational Rehabilitation (DVR) process oversight and management; upon admission collect and retain documentation to ensure competent surrogate decision making through Power of Attorney (POA) and foster the POA activation process as needed.
• Facilitate and foster the process for residents to secure long-term competitive integrated employment.
• Provide oversight to additional Resident Services Department staff, including Resident Advocate and Resident Life Coordinator.
• Work with families, guardians, and/or others as needed to resolve individual resident matters related to health insurance, Medical Assistance, Medicaid Part D, and other benefits.
• Develop and maintain positive relationships with all families, guardians, etc., keeping them informed regarding the health and wellbeing of their loved ones.

4. Administration
• Budget – Responsible to design, implement, monitor/analyze and control annual budget for functional area(s) of responsibility, taking expedient corrective action to ensure operations are within budget. Maintain a close working relationship with Finance Department, proactively communicating any changes to the status of resident(s) that have financial implications to the organization.

• Policy Development – Develop policies, protocols, and best practices to ensure consistency, resident safety, and optimize the quality of care and resident experience.
• Scorecard – Maintain a continuous improvement approach to work and the Resident Services department overall. Consistently achieve key performance indicator targets and financial metrics.

5. Ministry Growth
• Develop and execute a vision for the Resident Services Department that enhances and expands services and programs, and ultimately helps individual residents and the overall resident population achieve optimal health and wellbeing.

6. Regulatory
• Understand, effectively interpret, and ensure compliance with all CBRF and RCAC state statutes, DHS 83 and DHS 89, respectively.
• Complete the DHS 83.15 Administrator Course. Proactively counsel and advise Administration on legal or regulatory compliance matters.
• Lead organization-wide, required annual training on topics including, but not limited to, Resident Rights, Client Populations, and Challenging Behaviours.
• Play an active role in internal and state-led investigations related to abuse, neglect, lack of staffing, unsafe conditions, poor care, mistreatment, transfers, discharges, and caregiver misconduct.

Minor Duties (20% to 25% of Job):

  • Assist as needed with organizational fundraising events and other activities and events.
  • Activitely participate in, and lead when needed, special committees, task forces, etc.
  • Attend and actively participate in mandatory organizational meetings.
  • Other duties as assigned.
